Banner Biography   Ø      Born in Portland Oregon on January 17, 1957 while          father was attending University of Oregon School of Dentistry Ø      Raised in Burley, Idaho.  Great Grandfather Sam Banner settled this area in early 1900’s Ø      Graduated from Burley High School 1975 Ø      Graduated Utah State University 1982 with Bachelors of Science.  Major Biology Minor Chemistry Ø      Graduated Case Western Reserve School of Dentistry – Cleveland Ohio 1986 Ø      Has been Practicing General Dentistry for the past 15 years in Burley Idaho with his father Dr. Max S. Banner Ø      Built new clinic 1992 Ø      Dr. Kevin is certified by the State Board of Dentistry to perform I.V. Sedation Ø      Continuing education is Top Priority –  Dr. Kevin has accumulated well over 300 hours of continuing education in the last 3 years with emphasis on Temporomandibular joint disorders and functional orthodontics Ø      Attending residency with Dr. Terrence O’Shaughnessy in Las Vegas, Nevada.  Special Emphasis on TMJ diagnosis and treatment    Dr. Kevin D.
